comprehensive and accurate assessment of patients. + Evaluates the documentation of patient outcomes. + Enhances the dignity and integrity of the patient and upholds their rights. + Maintains confidentiality and ensures that patient and hospital information and records are not disclosed or misused in any way. + Coordinates and implements the intervention(s) identified in the plan of care / clinical path. + Evaluates and records progress towards expected outcomes and revises the plan of care as necessary. + Formulates and coordinates in conjunction with the Visiting Medical Practitioner (VMP) a plan of care, making substantive nursing judgements based on the health status, physical examination, preference, care and progress of each patient. + Communicates patient response to treatment and care and documents accordingly.* Facilitates communication between the patient, family, medical, nursing, ancillary and allied health staff.* Initiates referrals and utilises available resources for the purpose of comprehensive care to patients and families.* Appreciates the complexity of moral decision making, working cooperatively with other members of the health care team to resolve ethical and clinical issues arising in patient care* Participates in team and hospital activities as agreed with NUM* Maintain clinical registration and any required indemnity cover (if applicable).* Always work within approved scope of practice under supervision by more senior staff as appropriate.* Take personal responsibility for the quality and safety of work undertaken.* Take all necessary care and precautions when undertaking clinical procedures.* Complete annual competencies and required training.* Maintain skills and knowledge necessary to safely and skilfully undertake work.* Consult with peers and other experts and refer to other healthcare workers when appropriate and in a timely manner.* Participate in clinical risk management, patient safety and continuous quality improvement activities as part of day-to-day work.* All practices are consistent with the National Safety and Quality Health Service (NSQHS) Standards, EQUiP National and relevant professional practice and ethical standards and guidelines.* Protect the health and safety of self and others, complying with all health and safety related policies, procedures and directions.* Complete required Fire and Emergency Training annually.* Complete required Workplace Bullying and Harassment as required.* Collaborate and clearly communicate with patients / clients and the healthcare team.* Accountable for personal achievement of 100% competencies as per unit and organisational needs.* Liaise closely with the allocated Education Consultant or Clinical Coach to implement and progress own training and development.* Attendance required at HICMR Workshop every 2nd year.* Perform duties of the position to best of their ability and to a standard acceptable to SVHA.* Comply with all SVHA policies, procedures, by laws and directions.* Treat others with respect and always behave professionally and in accordance with the SVHA Code of Conduct.* Only access confidential information held by SVHA when this is necessary for business purposes, maintaining the confidentiality of that information once accessed.* Participate in the annual SVHA performance review process.* Display adaptability and flexibility to meet the changing operational needs of the business.* Display a willingness to develop self and seek to improve performance.* Complete other duties as requested.* Current immunity status that complies with SVHA immunisation policy.* Comply with applicable Enterprise Bargaining Agreement provisions (if applicable).* Ensures the provision of a safe work environment, work practices and minimises risks to self, staff and patients.* Maintains or exceeds standards of infection Control, WHS, Clinical Risk and legislative compliance.* Be accountable for personal knowledge of legislative requirements, policies and procedures in the areas of Infection Control, WHS, Clinical Risk.* Is accountable for ensuring all documentation is accurate and complete on every shift for duties allocated.* Ensures all work undertaken is within own capacity, experience and training and does not accept duties beyond own abilities* Takes immediate action to rectify any unsafe situations or acts, reporting all incidents and near misses according to hospital policy and procedures.* Strictly follows Manual Handling Policy.* Undertake work of a general nature under the direct supervision of a Grade 3 or Grade 4 Theatre Technician or Nurse Team Leader.* All new starters will be required to complete a mandatory orientation program for the first three months under the direct supervision of a Theatre Technician Grade 3 or 4.* Automatic progression to Theatre Technician Grade 2 will occur upon completion of Certificate III in Health Services Assistance (Operating Theatre Support) and a satisfactory Performance Appraisal.* National Criminal History Check.* Specialist qualifications and/or experience as appropriate to the area of work.* A valid working with children check.* Ability to maintain collaborative and effective relationships with all multidisciplinary team members.* Ability to work flexibly within the Facility service.* Ability to provide clinical practice and care consistent with the mission and philosophy of the facility.* Ability to plan and coordinate patient/resident/client care effectively, maintain patient/resident/client advocacy and utilise patient/resident/client management systems.* Ability to participate in and support education activities and to contribute to the development of self and other team members.* Have a working knowledge
