We are seeking a highly skilled and experienced Tuberculosis Care Coordinator to join our multidisciplinary team in Southern NSW. As a key member of the team, you will be responsible for delivering person-centered, specialist nursing care at an advanced practice level for TB patients in the region.

**About the Role**

* Deliver high-quality patient assessment, care planning and referral services.
* Coordinate TB clinics in partnership with tertiary providers and General Practitioners to ensure a flexible, quality TB management and treatment service.

**Key Responsibilities**
">
* Develop and implement individualized care plans that meet the unique needs of each patient.
* Collaborate with other healthcare professionals to provide comprehensive care and support services.
* Ensure timely and effective communication with patients, families, and healthcare teams.

**Requirements**
">
To be successful in this role, you will need to have:
">
* A current AHPRA registration as a Registered Nurse with post-registration qualifications in infection control, Public or Primary Community Health, or related field.
* At least three years' clinical experience and NSW TB Service accreditation to administer TST and BCG vaccinations.
* Competence in multidisciplinary team-based speciality nursing care; ability to exercise extended autonomy in clinical decision making and leadership in nursing practice advancement.
