Community Support Worker Opportunity
We are seeking a skilled and compassionate Community Support Worker to join our team in Broome, providing high-quality care and support to individuals in their own homes.
Key Responsibilities:
* Deliver person-centered care and support to clients with a focus on promoting independence and well-being.
* Work collaboratively with clients, families, and other healthcare professionals to develop and implement personalized care plans.
* Conduct thorough assessments of client needs and develop strategies to address these needs.
* Maintain accurate and comprehensive records of client interactions and progress.
Requirements:
* At least 1 year of experience in support work, aged care, or disability services.
* Certificate III or IV in Individual Support or Aged Care.
* Working rights in Australia.
