* Who we're looking for: Experienced Pharmacists proficient at medication reconciliation/review.
We are seeking a Palliative Care Pharmacist to join our team at Mater Hospital North Sydney, supporting a newly established palliative care unit. This is the first dedicated role of its kind at this site, offering a unique opportunity to contribute to the development of pharmacy services within palliative care.
About the Role
This role introduces dedicated clinical pharmacy support, strengthening medication management within this specialised area.
Key Responsibilities
* Conduct medication reconciliation and medication reviews
* Support symptom management strategies through medication optimisation
* Provide advice on pain management, end-of-life care, and deprescribing where appropriate
* Work collaboratively with medical, nursing, and allied health teams
* Support safe and effective medication use across the patient journey
* Educate patients, carers, and staff on medication use and best practice
* Assist access to specialist palliative care medicines via SAS
* Liaise with community team and hospitals to provide smooth transition of care.
Shift Information
* 19-hours a week
* Preferred days: Monday, Tuesday and Wednesday
* Flexible work pattern with 3x 6-hour shifts, or 4x shorter shifts
* Negotiable start/finish times
About You
You are a clinically capable Pharmacist with a passion for patient‐centred care, particularly in complex or supportive care settings.
Hospital experience is beneficial but not required; we welcome applications from pharmacists across community or other clinical settings who are looking to transition into hospital or palliative care‐focused practice.
Eligibility Requirements
* AHPRA Pharmacist registration with no restrictions on practice
* Australian working rights, valid for at least 12‐months from date of employment
* Demonstrated experience in clinical pharmacy services (medication review/reconciliation)
Desirable Skills & Qualifications
* Experience in hospital pharmacy
* Exposure to palliative care or complex patient cohorts
