Role Overview
We are seeking a skilled Social Care Specialist to join our multidisciplinary team, providing crisis intervention, psychosocial assessments and support to patients in an acute hospital setting.
This rewarding role involves working closely with patients, their families and other healthcare professionals to provide empathetic and expert care, ensuring smooth transitions through the healthcare system.
The ideal candidate will possess a strong understanding of the psycho-social impact of hospitalisation and be able to complete comprehensive psychosocial assessments and formulate effective discharge plans in a short timeframe.
Key Responsibilities
* Provide crisis intervention and psychosocial support to patients and their families
* Conduct thorough psychosocial assessments and develop discharge plans
* Collaborate with multidisciplinary teams to ensure seamless patient care
* Provide grief and loss support, as well as advocacy and case management services
Required Qualifications and Skills
* Degree in Social Work or related field with eligibility for membership of the AASW
* Proven experience working in an acute hospital setting or medical rehabilitation environment
* Excellent communication and interpersonal skills
* Ability to work effectively in a fast-paced, dynamic environment
