Role Overview:
The Primary Health Care Nurse Specialist will be responsible for providing holistic, culturally appropriate primary health care services to patients in need. This will involve conducting comprehensive assessments, developing and implementing patient care plans, and engaging in health promotion and disease prevention activities.
Key Responsibilities:
* Clinical Assessments: Conduct thorough clinical assessments to identify patient needs and develop effective care plans.
* Patient Care Plans: Collaborate with multidisciplinary teams to develop and implement patient care plans that address individual patient needs.
* Health Promotion: Engage in health promotion and disease prevention activities to improve overall patient outcomes.
* Continuous Improvement: Contribute to the continuous improvement of clinical practices and protocols to enhance patient care.
Qualifications and Experience:
The ideal candidate will have a Master's degree in nursing, registered nurse qualification, and demonstrated experience in delivering primary health care services. Additionally, the candidate should have a commitment to working in a culturally sensitive and inclusive manner.
