We are seeking an experienced and compassionate Clinical Care Partner to provide clinical leadership, case management and care coordination across our Home Care services. This is an exciting opportunity for a Registered Nurse or Allied Health professional who is passionate about person-centred care, quality outcomes and leading others.
About the Role
Reporting to the Home Care Nurse Consultant, the Clinical Care Partner leads personalised care planning and oversees the safe delivery of services for a portfolio of clients.
You will work closely with clients, families, Care Partners, Team Leaders and healthcare providers to ensure care remains responsive, compliant and aligned to each client's goals.
Desired Skills and Experience
Clinical Assessment & Care Planning
* Conduct comprehensive client assessments covering physical, cognitive and psychosocial needs
* Develop personalised care plans aligned to goals, preferences and cultural considerations
* Review and approve care plans developed by Care Partners
* Monitor for health deterioration and coordinate timely interventions or referrals
* Reassess and update care plans as needs or funding arrangements change
Care Coordination & Leadership
* Coordinate services with GPs, specialists, allied health providers and internal teams
* Support clients to navigate My Aged Care, Home Care Packages and Support at Home programs
* Provide leadership, coaching and clinical guidance to Care Partners
* Support Team Leaders to monitor Home Care Support Workers and safe service delivery
* Manage purchases of prescribed aids and equipment
Client & Family Support
* Educate clients and families on care plans, service agreements and available supports
* Promote culturally safe, inclusive and trauma-informed care
* Assist with future care planning and changing support needs
Governance, Quality & Compliance
* Maintain accurate documentation and clinical records
* Participate in case reviews, audits and continuous improvement initiatives
* Manage complaints, incidents and risk escalations appropriately
* Support compliance with aged care standards and regulatory requirements
About You
You are clinically capable, relationship-focused and thrive in a fast-paced environment where no two days are the same.
* Tertiary qualifications with current AHPRA registration as a Registered Nurse or relevant Allied Health discipline in Australia
* Current driver's licence and access to a comprehensively insured vehicle for work use
* Strong clinical assessment and case management capability
* Experience supporting older people living at home
* Strong knowledge of ageing, dementia and behaviour support strategies
* Excellent communication, stakeholder engagement and problem-solving skills
* Strong organisational and time management skills
* High level computer literacy including Microsoft Office
* Minimum 3 years' experience in a similar role
* Additional specialty qualifications relevant to community or aged care
* Strong relationships across medical, allied health and community networks
