About The Role

The Mobile Assertive Treatment Team (MATT) is one of two assertive community treatment teams within Sydney Local Health District (SLHD). Based at Camperdown Community Mental Health Centre, MATT provides tertiary-level community mental health care to people living in the Camperdown, Marrickville and Redfern catchment areas.

MATT is a multidisciplinary team that delivers assertive outreach services to people with severe and enduring mental illness who require intensive, community based support to remain well and living independently. Assertive outreach involves proactive and flexible engagement strategies, including home visits and collaborative care planning, to support consumers who may struggle to access traditional clinic based services.

The service operates seven days a week, with extended hours to ensure support is available at times that best meet the needs of consumers. The model enables timely, responsive, and person centred care delivery outside of standard business hours.

Role

As part of the team you will bring your discipline specific expertise to an integrated model of care, working alongside nursing, medical, social work, occupational therapy, psychology and peer work. Your role will involve:
* Building strong therapeutic relationships with consumers and their supports.
* Providing individualised interventions to improve mental health, physical wellbeing, and psychosocial functioning.
* Supporting consumers to achieve their recovery goals and enhance their quality of life.
* Coordinating care across services, including housing, health and social supports.

MATT provide coordination and clinical services for identified housing support programs.

Ideal Candidate
* Recognised tertiary qualifications appropriate to profession. Full registration and eligibility for membership to professional association, as appropriate.
* Demonstrated competencies to provide contemporary mental health practice.
* Current NSW Drivers Licence (P1 and P2 acceptable)
