Your New Role
As a Clinical Nurse Consultant – Advance Care Planning, you will provide expert clinical leadership and advanced nursing practice across Metro South Health. This role is central to delivering safe, compassionate, and person-centred care, while leading service development, education, quality improvement, and research within the Advance Care Planning specialty.
You will work collaboratively with multidisciplinary teams, patients, carers, and stakeholders to influence best practice outcomes, embed evidence-based care, and support high-quality end-of-life and future care planning across the health service.
Key Responsibilities
* Provide advanced, expert nursing care and clinical consultancy in Advance Care Planning
* Lead and role model person-centred, evidence-based practice across the care continuum
* Deliver clinical leadership, education, mentoring, and support to nursing and multidisciplinary teams
* Apply critical thinking, advanced assessment, and expert clinical judgement to improve patient outcomes
About you
You are an experienced and confident Registered Nurse with a passion for Advance Care Planning and person-centred care. You bring strong clinical leadership, advanced communication skills, and the ability to influence practice at both a clinical and system level. You thrive in collaborative environments and are committed to improving patient experiences and outcomes through evidence-based care.

* Proven ability to deliver high-quality, person-centred care
* Strong leadership skills with experience in mentoring, education, and clinical supervision
* Ability to work autonomously while contributing to high-functioning multidisciplinary teams

About us
The Metro South Palliative Care Service (MSPCS) provides specialist palliative care utilising the model for populated-based care developed by Palliative Care Australia. MSPCS provides consultative and direct care for patients in all hospitals in Metro South District and provides community-based specialist palliative care across the district. Community services are based at Brisbane South, Redland, and Logan. The MSPCS team is an interdisciplinary group of healthcare professionals who deliver collaborative patient-centred care in partnership with acute, domiciliary, aged health care providers and general practitioners.
