**Position Overview**

We are looking for dedicated In-Home Care Support Workers to provide high-quality, person-centered support to NDIS Participants. Your role will be crucial in helping them achieve their goals, enhance their quality of life, and improve their physical and mental health.
**Key Responsibilities**
As an In-Home Care Support Worker, your responsibilities will include:
- **Individualised Support**: Providing a range of services tailored to meet the unique needs of NDIS Participants, such as housekeeping, grooming, toileting, meal preparation, medication management, and more.
- **Knowledge and Care**: Demonstrating an understanding of health and well-being, cultural plans, transition plans, and behavior support plans. You will also be knowledgeable about protocols regarding restrictive practices and medication requirements.
- **Community Inclusion**: Assisting Participants in participating in community activities and services based on their preferences and abilities. You will help them establish and maintain relationships and interests.
- **Communication**: Providing regular feedback to the Home and Community Support Manager regarding issues that require follow-up. This includes matters related to Participants, their families, external supports, fellow employees, and community access activities.
- **Compliance**: Adhering to UFC's Code of Conduct, policies, and Quality Management System procedures to ensure the highest standards of care and support.
- **Safety and Wellbeing**: Demonstrating a strong commitment to the safety and well-being of yourself, Participants, and others in the work environment.
**About You**
You will thrive and succeed in our team by bringing the following experience to this position:
- Certificate III or IV in individual supports (aged care, disability or home and community services)
- Drivers License and vehicle
- Experience working within the In-Home Care environment
- Ability to meet deadlines, work with mínimal supervision and pressure.
- Effective written and verbal communication skills and a demonstrated ability to write clear case notes.
- Understanding of WHS procedures
**Desirable**:
- Qualification in community services, disability, mental health or related field
- Qualifications in nursing, assistant nursing or registered nurse
- Manual Handling
- WWC
- Experience working with CALD clients
- Complex care
- Dementia care
- Epilepsy Trained/Experienced
- Mental Health first aid
- Certificate IV in mental health
- Trauma informed care
- Mental health awareness and sensitivity training
- NDIS Worker Screening Clearance
- Current First Aid Certificate
- **Unrestricted Australian Driver's License and vehicle with Comprehensive Insurance
