It connects individuals with services across Peninsula Health and the broader community, using health coaching to deliver targeted therapies and foster community connections. nThe Coordinator works with each client to create a personalised Action Plan based on their goals. They provide professional interventions as required and serve as the primary point of contact for the client throughout an estimated six-month period. This includes delivering and coordinating services across Peninsula Health and the community, with outreach in homes and community settings. nThe role involves integrated care planning, motivational interviewing, and data collection to ensure safe, evidence-based practices and meet key deliverables for the pilot program. The Coordinator also contributes to establishing this innovative program while operating within professional standards and enterprise agreements.
