Are You the Right Fit?
We are looking for someone who thrives in individual and autonomous practice while enjoying the safety and support of a team environment. In this role, you will facilitate and implement health assessments and Health Management Plans for children and young people entering Out of Home Care. You will work confidently within a multidisciplinary and multi-agency approach, collaborating with key stakeholders both within SWSLHD and externally, including DCJ, Department of Education, GPs, private health providers, NGOs, and carers. Strong clinical, liaison, implementation, and monitoring skills are essential, along with a passion for improving health outcomes for vulnerable children and young people.
Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
* A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).
* Demonstrated experience in a health related field and ability to provide a range of Social Work interventions to children, young people and their carers/families.
* Demonstrated clinical knowledge and skills in psychosocial aspects of heath, illness, injury and recovery.
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
