We're currently accepting applications for a CLS Case Worker position. As a Case Worker, you'll be responsible for providing hands-on assistance to individuals in the Community Living Support (CLS) program. Your role involves implementing the core values of Open Minds and using Motivational Interviewing and Acceptance and Commitment models as a framework for service delivery. You'll collaborate closely with colleagues, team leaders, and other service providers to ensure efficient service provision. Additionally, you'll track and report service outcomes against evaluation criteria and key performance indicators to measure the effectiveness of the support provided.
Join us as a CLS Case Worker in Lismore, with opportunities to work across Northern NSW. This is a full-time maximum term contract until 31st March 2024 with possibility of extension.

How you will make a difference:
Service Delivery:
* Meet with clients to discuss their needs and develop a personal support plan.
* Liaise with appropriate stakeholders about progress with the participants support plan.
* Build and maintain positive and professional relationships with clients, colleagues, and other stakeholders ensuring that role and professional boundaries, confidentiality, and cultural sensitivities are respected.
* Identify and assist in locating resources, services, and supports that facilitate community integration including social, educational, vocational and leisure activities.
Administration and Reporting:
* Timely and accurate communication of important details of work to Team Leaders and colleagues particularly, difficulties with progress, adverse events, potential risks, and significant changes to risks.
* Prepare for monthly supervision with Team Leader and provide information as required to facilitate the completion of internal and external reports.
Who you are:
Education and qualifications
* Professional degree or Diploma (Mental Health, Community Sector, Case Management, Counselling or related)
Essential skills, knowledge and experience
* Significant relevant experience working within the health, mental health, community, or social service sectors
* Open to learning new approaches to mental health work and willing to actively engage in continuous improvement opportunities
* Experienced & passionate about working in a team committed to making a positive change in the lives of people with severe, complex, and persistent mental illness.
* Highly developed verbal and written communication, negotiation & conflict resolution skills and the ability to engage with people with mental health difficulties, their families and other service providers and partner agencies.
* Capacity to work independently and collaboratively being accountable for the effective delivery of services.
* Capacity to understand role and the boundaries
