We are seeking a highly skilled Social Worker to join our multidisciplinary team in delivering safe, effective, and patient-centred care in the Statewide Mental Health Services.
The Team's Goal
Our team works together to provide high-quality mental health services, ensuring timely and safe discharges by addressing complex social needs, promoting patient flow, reducing delayed discharges, and preventing avoidable readmissions.
Your Expertise
You will play a critical role in ensuring that patients have the right support in place to maintain their recovery in the community, reducing the risk of relapse.
Key Responsibilities
* Deliver quality care to consumers of the Inpatient and Extended Treatment Mental Health Service based on best practice principles and within a collaborative framework.
* Provide specialist assessment and treatment service to consumers of Inpatient and Extended Treatment Mental Health Service and their families or carers.
* Promote community awareness in relation to mental health and act as a consultant to other agencies with regard to the support and management of consumers with mental health needs.
Requirements
* Tertiary qualification in Social Work approved by the Australian Association of Social Workers (AASW)
* Eligibility for membership with the Australian Association of Social Workers (AASW)
* Overseas qualified Social Work candidates require additional qualifications.
