What you will do As assessment & transition coordinator – mental health, you will support people in custody with mental health needs. You will help them plan for release and connect to services. You will: deliver targeted support to people with serious mental health needs build strong links with health, drug and alcohol, and community services create clear release plans that support safe return to the community complete accurate risk and needs assessments and support case reviews manage reliable data and share updates with the wider network. About you A tertiary qualification in a related field is desirable. Experience in human services or welfare work is highly regarded. You should also have: a solid understanding of mental health and reintegration needs a trauma-informed approach and care for complex clients the ability to work well with Aboriginal communities and diverse groups strong computer, data and communication skills.
