About the Role:
Promote independence and wellbeing for older community members and their carers through collaborative support and advocacy.
* Evaluate client and carer needs in partnership to develop tailored solutions
* Coordinate timely referrals and service delivery within programme guidelines
* Maintain accurate records, documentation, and reporting for quality assurance
* Support carers through education, information, and conflict resolution to enhance their capacity and resilience
* Communicate effectively with subcontractors, allied health providers, and other stakeholders to ensure seamless delivery of services
* Contribute to team initiatives and facilitate group events that foster connections and community engagement
Essential Skills and Qualifications:
A qualification in welfare, social work, nursing, or a related field; advanced communication and interpersonal skills; strong organisational and time management abilities; excellent computer and reporting skills; and a current Victorian driver's licence.
Desirable Experience:
Previous experience in community-based services, carer engagement, and multidisciplinary teamwork would be highly regarded.
