Qualifications and Licences
* Cert IV in Individual Support (Disability) or equivalent
* Current first aid & CPR
* Valid Driver's Licence
* Current National Police Clearance(or willing to obtain)
* Evidence of right to work In Australia if on a visa
* Current NDIS Worker Screening Check
* Current Working with Children Clearance(or willing to obtain)
* Access to a smartphone
Experience and/or Specialist knowledge
* Previous experience in the disability sector or a related sector including; knowledge of providing participant choice and self-directed supports
* Positive communication and interpersonal skills including the ability to work and communicate effectively with participants, their families, community members and other agencies
* Demonstrated effective time management and prioritising skills, whilst maintaining flexibility for unexpected tasks
* Ability to write basic reports and use of computers
Desirable
* Experience with participants who have complex support needs is viewed favourably
* Experience in working with Indigenous Australians and people from culturally and linguistically diverse background
* Experience with any of the following will be highly regarded - ASD, Down Syndrome, Mental health, Personal care, Medication management, hoisting, enteral feeding, and stoma care.
Tasks and Responsibilities
* Support program participants to participate in age-appropriate activities and daily living tasks, including in-home support and access to activities and outings in the community
* Provide direct care assistance (including personal care) to each participant in accordance with their service plan/agreement
* Work in accordance with client right's policies including, privacy, confidentiality, dignity of risk, right to be free from abuse and neglect and restrictive practices
* Take reasonable care to avoid foresee able risk without unduly limiting the ability of service users to take responsibility for his/her decisions
* Ensure Individual Plans/goals, Service Agreements and NDIS Plans for each participant are implemented by all staff
* Participate in the planning process for individual participants and monitor outcomes
* Work with participants, family/carers, relevant case managers and other professionals to support the participants individual goals
* Responsible for maintaining a balanced and efficient staff roster
* Review staff timesheets before submission to payroll
* Ensure the allocated budget is accurate and all expenditure is receipted and recorded
* To be responsible for the effective operation of the service, including supervision and delegation to house staff
* Monitor and feedback the status of staff performance to the Human Resources Manager as part of the performance appraisal process
* Ensure participant and staff records are maintained to an informative standard, including adhering to the National Quality &Safeguarding framework in relation to recording and reporting of incidents.
* Administration of medication in accordance with support plans as required
* Effectively communicate relevant information to management, staff, program participants, families and carers and attempt to resolve any issues raised demonstrating empathy and responsibility
* Responsible for the facilitation/chairing/minute taking of team or house meetings
* Communicate information regarding participant planning and behaviour management strategies as required
* Develop and maintain professional relationships with all stakeholders to promote positive outcomes for participants in relation to their physical, emotional and mental health wellbeing
* Daily use of computer systems and software programs to ensure effective service delivery, communication and storage of information
* Implement behaviour support plans under the instruction of coordinators and managers
* Maintain, update and review participant files
* Other documentation and report writing as required
* Undertake a range of active and physically demanding activities with program participants
* Operate a variety of disability care equipment, including vehicle transportation of participants
* Respond to changes in participant and stakeholder needs
* Participate in essential training, meetings and supervision as required as well as identify additional professional development required to progress in this position
