Lead Aged Care Practitioner
We are seeking a highly skilled and compassionate practitioner to lead the assessment of older adults in need. As an expert in aged care, you will play a vital role in connecting individuals with essential services that promote independence and well-being.
Key Responsibilities:
* Conduct thorough assessments to identify the needs of older adults
* Develop comprehensive care plans to support independent living
* Collaborate with families and carers to ensure effective communication and engagement
* Ensure all assessments are conducted in accordance with Department of Health contractual requirements

Requirements:
* At least 2 years' experience in aged care, including client assessments and/or service delivery
* Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work)
* Current unrestricted General Registration with AHPRA or relevant professional association
* Proficiency in Microsoft Office and client management systems
* Valid Australian driver's license and ability to travel within designated service areas
