The Older Persons Mental Health Service provides multidisciplinary community mental health assessment and treatment services for consumers affected by mental illness aged over 65 years, and their family/significant other and carers. The service is recovery oriented, person centred, strengths based, collaborative and empowering; and operates on the premise that consumers can and do recover from mental illness.
**Key Responsibilities include**:

Contribute to positive patient outcomes by;
- providing supervision, coordination and leadership to effectively assess, plan, manage and evaluate patient outcomes at an advanced practitioner level particularly for patients with unpredictable outcomes
- using evidence based clinical practice that maintains patient safety
- leading management and coordination of more complex patients
- facilitating accurate and timely communication to drive effective multidisciplinary team functioning
- role modelling best practice in clinical and professional domain.
