Job Ad Summary Synergeer Care, a registered NDIS provider based in Redcliffe QLD 4020, is seeking an experienced Disabilities Services Officer to join our multidisciplinary team. In this role you will assess the needs of NDIS participants and people living with disability, plan and implement individualised support programs, and coordinate services to promote independence, community participation and recovery. This is a permanent full-time position offering a salary of $80,000 - $85,000 per annum plus superannuation. We deliver disability, mental health, nursing and home care services to NDIS participants, inpatient and community mental health consumers, and the elderly across the greater Brisbane and Moreton Bay regions. Our practice is grounded in a recovery-focused and trauma-informed model of care. We believe in empowering every individual we support to achieve their goals, build capacity and participate meaningfully in their community. Our multidisciplinary team includes nurses, allied health professionals, support coordinators and disabilities services officers who collaborate to deliver holistic, person-centred services. Duties and Responsibilities Reporting to the Service Manager, the Disabilities Services Officer will be responsible for the following duties: Client Assessment and Needs Analysis — Interview NDIS participants and other clients to assess the nature and extent of their difficulties, functional capacity, goals and support requirements, using evidence-based assessment tools and a person-centred approach. Program Planning, Development and Implementation — Plan, develop and implement individualised educational, training and support programs aligned with each participant's NDIS plan goals, including skill-building programs that promote independence, social participation and community access. Monitoring, Reporting and Review — Monitor and report on the progress of clients against their individual goals and NDIS plan objectives; maintain accurate case notes, progress reports and outcome data; and conduct regular reviews to adjust programs as required. Referral and Service Coordination — Identify when clients require additional or specialist assistance and refer them to appropriate agencies, allied health professionals, government services or community organisations to ensure comprehensive support. Community Needs and Resource Assessment — Assess community need and available resources for health, welfare, housing, employment, training and other facilities and services relevant to people living with disability and mental health conditions in the Redcliffe and Moreton Bay region. Stakeholder Liaison and Community Engagement — Liaise with community groups, welfare agencies, government bodies (including the NDIA) and private businesses about community issues affecting people with disability; promote awareness of community resources and services available to participants and their families. Family and Carer Support — Support families and carers by providing information, education and guidance on disability-related matters; facilitate family involvement in goal-setting and program planning in accordance with the participant's wishes. Funding Submissions and Reporting — Prepare submissions for funding and resources, including NDIS plan review reports and supporting documentation; prepare reports to government bodies, the NDIA and other agencies as required. Compliance and Quality Assurance — Ensure all service delivery complies with the NDIS Practice Standards, NDIS Code of Conduct, relevant legislation and Synergeer Care's internal policies and procedures; participate in quality improvement activities and audits. Recovery-Focused Practice — Apply recovery-focused and trauma-informed principles in all interactions with participants, ensuring that services are delivered in a manner that respects individual dignity, choice and control. Qualifications and Requirements To be considered for this position, applicants must be able to demonstrate the following: Essential An AQF Associate Degree, Advanced Diploma or Diploma (ANZSCO Skill Level 2) in a relevant field such as Community Services, Disability Services, Mental Health, Social Work or a related discipline — or at least three (3) years of relevant experience in lieu of formal qualifications, in accordance with ANZSCO guidelines Demonstrated experience assessing client needs and developing, implementing and reviewing individualised support programs for people living with disability Experience monitoring client progress, maintaining case records and preparing reports for funding bodies or government agencies Demonstrated commitment to person-centred, recovery-focused practice and an understanding of trauma-informed care principles Knowledge of the National Disability Insurance Scheme (NDIS), including the NDIS Practice Standards, NDIS Code of Conduct and participant plan processes Strong interpersonal, communication and stakeholder engagement skills, including the ability to liaise effectively with clients, families, community organisations and government bodies Current NDIS Worker Screening Check (or willingness to obtain prior to commencement) Current Australian driver's licence Eligibility to work in Australia Desirable Experience working within a registered NDIS provider organisation Knowledge of community resources and services available in the Redcliffe, Moreton Bay and greater Brisbane regions Current First Aid and CPR certification Current Working with Children Check (Blue Card) or willingness to obtain Salary $80,000 - $85,000 per annum plus superannuation (currently 12%), commensurate with qualifications and experience.
