Position Overview:
We are seeking a passionate and dedicated Social Worker to join our team as a Care Coordinator. The role would initially be specialising in veteran services with the plan to branch out further. The ideal candidate will have experience in providing social work services in a healthcare setting, with a strong understanding of the unique needs of veterans and client's in the healthcare system. The Care Coordinator will play a critical role in ensuring our clients receive seamless and coordinated care, addressing both medical and psychosocial needs.
Key Responsibilities:
* Conduct comprehensive assessments of veteran patients to identify their physical, psychological, and social needs.
* Develop, implement, and monitor individualised care plans in collaboration with on-site healthcare providers, clients, and their families.
* Coordinate and facilitate access to healthcare services, community resources, and support networks.
* Advocate for clients to ensure they receive appropriate and timely care.
* Provide counselling and support to veterans and their families, addressing issues such as PTSD, substance abuse, and other mental health challenges.
* Assist patients with navigating the health system and accessing veteran-specific benefits and services.
* Maintain accurate and up-to-date patient records and documentation.
* Participate in multidisciplinary team meetings to discuss patient care plans and progress.
* Conduct outreach and education activities to increase awareness of available resources and support for veterans.
* Stay informed about best practices and advancements in veteran care and social work.
* Assisting in marketing and advocating services to the community
Qualifications:
* Bachelor or Masters degree in Social work
* Full Australian working rights

Pay: $32.89 – $40.48 per hour
