In addition to base salary, all roles attract 12% Superannuation + Salary Packaging

An exciting opportunity exists for an experienced Allied Health Professional (Level 4) or NUM 1 to lead the Transitional Aged Care Program (TACP) in Bowral. This leadership role provides responsibility for the clinical and operational management of a multidisciplinary team delivering safe, effective and responsive transitional aged care services across the Wingecarribee region. Key accountabilities include oversight of clinical governance, intake, case management, service delivery and discharge planning, as well as workforce planning, service performance, and continuous improvement initiatives. The role supports older people to transition safely from the acute hospital setting to the community, contributing to NSW Health's commitment to high‑quality, person‑centred care.
Are You the Right Fit?
The ideal candidate is a suitably qualified Allied Health Professional or NUM with demonstrated experience in leading multidisciplinary teams within aged care or community health settings. Strong capability in clinical governance, case management and operational leadership, along with well‑developed communication and organisational skills, is essential to effectively manage service demands and stakeholder relationships. A sound understanding of the aged care system, relevant legislation, professional standards and NSW Health policies is required. The role suits a values‑driven leader who models behaviours aligned with the NSW Health CORE values, supports staff performance and development, and drives service improvement to achieve positive outcomes for clients, carers and the broader health system.

* Recognised degree in a health related field (Nursing or Allied Health Professional) with current professional registration or eligibility for membership of relevant professional body as relevant.

* Can you describe your relevant experience in coordinating and supporting a team to effectively manage a complex caseload?
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
