**Job Title:** Mental Health Professional
About the Role:
This is an exciting opportunity for a skilled and motivated professional to join our multidisciplinary team at Forster Community Health Centre. As a Mental Health Professional, you will be responsible for providing high-quality assessment, care coordination, intervention, and follow-up for adults experiencing mental health concerns.
You will work collaboratively with consumers, their families/carers, GPs, Community Managed Organisations (CMOs), and other service providers to ensure safe, effective, recovery-focused care.
Key Responsibilities:
* Provide comprehensive mental health assessments and develop individualized care plans
* Collaborate with healthcare teams to coordinate care and services
* Conduct interventions and provide support to individuals and their families
* Develop and maintain relationships with community partners and stakeholders
Requirements:
To be successful in this role, you will need:
* A recognised and accredited tertiary qualification in social work, psychology, occupational therapy, dietitian, or nursing
* Registration or eligibility for registration with the relevant board via AHPRA
* Excellent communication and interpersonal skills
* A commitment to ongoing learning and professional development
