Position Overview
The Care Coordinator Integrated Team Care (ITC) is responsible for orchestrating comprehensive care for clients with chronic and complex health conditions. This role emphasizes a managed approach to coordinating client care activities and facilitating effective communication among health service providers to ensure optimal health outcomes.
As part of the Integrated Team Care Coordination, this position aims to enhance access to culturally appropriate chronic disease care for Aboriginal and Torres Strait Islander families, ensuring they receive the necessary support from allied health and specialist services.
Candidate Profile
The ideal candidate will possess a strong commitment to improving health outcomes for Aboriginal and Torres Strait Islander communities and demonstrate the following qualifications and skills:
* Relevant qualification as an Aboriginal Health Practitioner Certificate IV or a Registered Nurse with current AHPRA registration.
* Experience working with Aboriginal and/or Torres Strait Islander communities, with a deep understanding of their cultures.
* Proficiency in the Microsoft Office Suite and familiarity with medical databases such as Pracsoft and Medical Director.
* Excellent interpersonal skills, with the ability to communicate effectively with diverse groups.
* Ability to function within a multidisciplinary team and independently, exhibiting leadership and coordination skills.
* Have a good understanding of the health system, including referral pathways.
The candidate must also possess a current 'C' Class Driver's License (Queensland) and be willing to undergo a Working with Children Blue Card and an Australian Federal Police check.
