hands-on care, education, and support
that empowers clients to stay safe, well, and independent in their own environment. Every visit is unique, and every client's story matters.
What You'll Do
Provide clinical nursing care in the home — wound care, catheter and stoma management, diabetes support, and chronic disease management.
Complete health assessments and develop personalised care plans.
Collaborate with GPs, carers, and allied health professionals for truly holistic support.
Educate clients and families, giving them confidence to manage their health.
The Role at a Glance
Type: Permanent Part-Time (4 days per week, Tue–Fri)
Shifts: Daytime only – no nights or weekends
Location: Taree/Forster and surrounding areas – community-based care with home office flexibility
Support: Ongoing clinical guidance, peer support, and autonomy to plan your day
Resources: iPad, laptop, phone, company vehicle + paid travel time
What We're Looking For
Current AHPRA registration (Division 1 RN)
Minimum 1 year Experience in community, aged care, or acute settings
Robust communication and time management skills
A passion for person-centred, community-based care
Valid driver's licence, Police Check, WWCC, CPR
Willingness to travel for client care for up to 2 hours
