About the Role
We are seeking a skilled and compassionate Case Manager to join our team. As an Outreach Case Manager, you will work closely with individuals and families at risk of losing their tenancy to provide vital support and guidance.
Key Responsibilities:
* Work outreach-style, meeting clients in their communities or homes to build trust and rapport.
* Manage a caseload, supporting clients to stabilise and sustain their housing.
* Complete strengths-based risk and needs assessments, and develop personalised support plans.
* Advocate for tenants, connect them to specialist services, and work collaboratively to remove barriers to housing stability.
* Help clients maintain tenancy agreements, meet landlord obligations, and prevent eviction.
About You
We are looking for a candidate with:
* A minimum Cert III in Community Services (or related field).
* Demonstrated experience in case management or outreach roles, particularly with at-risk individuals.
* High emotional intelligence, professionalism, and the ability to work both independently and in a team.
* Confidence working in public spaces, community housing settings, and private residences.
* A current Driver's Licence and valid Blue Card (Working with Children Check).
* A passion for social justice and a commitment to dignity, empathy, and empowerment.
