We're expanding and are looking for a dedicated Enrolled Nurse to join our Access Aged Care team within the eastern suburbs/hills of Perth.

What You'll Do Provide clinical management and coordination across residential aged care facilities Conduct regular resident reviews and support person-centred care plans.
Work closely with a multi-disciplinary team, supported by a Nursing Regional Manager and admin staff.
Build strong relationships with residents, families and facility partners.
Use your clinical knowledge and IT skills to ensure quality outcomes.
What You'll Bring Diploma of Nursing and AHPRA registration.
A minimum of 18 months Nursing experience (leadership experience highly regarded)Strong organisational, problem-solving and communication skills.
Confidence with IT and documenting resident needs.
A driver's licence and reliable vehicle.
