Clinical (RN) Case Manager x 2 positions
We are a rapidly growing organisation that is doing extraordinary things for those living with a disability. We don't use agency staff. We build a team of nurses and support workers around our clients to ensure consistency of care, and our employees have regular shifts with the same clients, so they build that relationship. We have an influx of clients and need an additional Clinical (RN) Case Manager rockstar to join our team. Our clients can be in the complex clinical health space, so we need your clinical expertise. As a Case Manager you play a vital role in the management of our clients and their supports. The role is based at Head Office in Southport, Monday – Friday, 8.00am to 4.06pm, with a wonderful team in Head Office and company culture rewards for effort.
Our model works best with you being an excellent RN with solid nursing experience. As the care coordinator/case manager for our complex clients you provide clinical oversight along with mentoring and supporting our community team of nurses and support staff. The role involves a mix of in-office work and community visits, training your support team, undertaking assessments and onboarding new clients. Company vehicles are available for use. We value client satisfaction over KPIs and invest in training and mentoring for the right person. We also love dogs in the office.

Responsibilities
* Provide clinical oversight and mentoring for our community team of nurses and support staff.
* Manage complex client cases and coordinate supports across multidisciplinary teams.
* Undertake comprehensive health assessments and manage conditions such as diabetes, catheter care, PEGs and wounds.
* Advocate for clients, maintain strong relationships with clients and liaise with multidisciplinary teams.
* Visit clients in the community, train the support team, undertake assessments and assist with onboarding new clients.
* Participate in the On Call roster (approximately every 8–10 weeks) with a generous on-call allowance after settled in the role.
* Work with the Head Office team and contribute to a stable, consistent care model.
Qualifications
* Qualified Registered Nurse with preferably 2-5 years experience.
* Previous case management and/or supervisory experience is desirable, but not essential.
* Ability to work collaboratively in a multidisciplinary team.
* Highly developed verbal and written communication skills.
* Confidence in managing time effectively, including the ability to work autonomously and as part of a team.
* IT savviness.
* Blue card and NDIS Workers Screening Clearance or willingness to obtain.
* Full, unrestricted AHPRA registration and legal working rights in Australia.
* Remuneration paid above the Nurses Award rate.
* Be available to participate in the On Call roster, once fully settled into the role.
* Note: Non-clinical preference related to dog-friendly office environment.
