about the role
The Case Manager: Tenancy Transition (Post‑Custodial Support) plays a key role in supporting clients as they prepare for release and reintegrate into the community. The position assists individuals in securing safe and sustainable housing, advocates with landlords and housing providers, and delivers practical coaching in essential tenancy skills such as budgeting, rent management, and property upkeep.
By working collaboratively with corrections, support services, and housing agencies, the case manager ensures that each client’s housing plan aligns with their recovery goals, supports adherence to AOD treatment, and contributes to reducing recidivism through stable, well‑supported community reintegration.
Desired Skills & Experience
* Relevant VET qualification.
* Or tertiary qualification in relevant field.
* Experience working with people exiting custody, or with individuals experiencing homelessness, AOD concerns, mental health challenges, or complex support needs.
* Strong understanding of tenancy frameworks, housing pathways, and the rights and responsibilities of tenants and landlords.
* Demonstrated ability to advocate effectively with housing providers, real estate agents, and community services.
* Skills in delivering practical coaching in tenancy‑related life skills, including budgeting, rent management, and property care.
* Experience supporting clients to engage in AOD treatment, relapse‑prevention planning, and recovery‑oriented supports.
* Ability to work collaboratively with corrections, community organisations, and multidisciplinary teams to support coordinated release planning.
* Knowledge of factors contributing to recidivism and strategies that promote successful reintegration.
* Strong case management skills, including assessment, goal planning, documentation, and risk management.
* Excellent communication, rapport‑building, and motivational interviewing skills.
* Computer literacy including understanding of MS Office Suite and use of Client Management Systems.
* Hold a current NSW Driver's License.
* Hold or have the ability to obtain a current Criminal History Check and Working with Children clearance for paid employment.
