Commonwealth Home Support Program (CHSP) Coordinator
About the Role
As the CHSP Coordinator, you'll play a pivotal role in designing and delivering service packages that are commercially minded, creative, and client focused.
You'll think outside the box to tailor solutions that are affordable, effective, and high-quality, ensuring our clients receive the best possible support.
You'll also bring solid administrative capabilities to the role, confidently managing billing, co-contributions, and reporting in line with government guidelines.
Your key responsibilities will include:
Building and maintaining trusted relationships with clients, RAS assessors, referral agencies, and allied health professionals.
Creating and coordinating service packages that meet client needs while aligning with CHSP targets and funding requirements.
Monitoring service outputs and ensuring compliance with CHSP guidelines.
Managing bookings, billing, and reporting with accuracy and efficiency.
Supporting clients with complex needs and escalating feedback appropriately.
Promoting Lutheran Services as a provider of choice within the local community.

About You
You're a proactive and solutions-focused qualified who thrives in a dynamic setting.

Experience in aged care, home care, or case management.
Strong administrative and customer service skills.
Excellent communication and stakeholder engagement abilities.
Knowledge of CHSP guidelines and aged care legislation, or a willingness to learn.
The ability to work autonomously and manage competing priorities.
A current driver's licence and a valid police check.
