Remuneration : $112,396 - $116,038 per annum, pro rata - dependent on qualifications

About The Role
The Pathways to Community Living Initiative (PCLI) is a coordinated state-wide approach to supporting people with severe and persistent mental illness and complex needs who have had long hospital stays, are at risk of long stays or experience frequent admissions and to re-establish their lives in the community.
Working in partnership with multidisciplinary mental health teams and developing partnerships with community managed organisations, primary health care and other government and non-government organisations to provide a collaborative approach to development of models, pathways and access to clinical care through PCLI. Promoting seamless integration and coordination of day-to-day work practices and will provide advice, assessment and support to multidisciplinary teams.
