About the Role
Step into a role where you support patients and families to navigate change, build resilience and improve wellbeing across our CALHN sites.
Deliver social work consults and counsel patients and families
Guide and support junior social workers and students
Drive quality improvement and service planning
Work closely with a multi-disciplinary team
Link patients with referrers, community services and follow-up care
About You
You'll thrive in this role if you bring strong clinical capability, compassion and a commitment to high-quality, patient-centred care.
Hold a social work degree eligible for AASW membership
Deliver skilled assessment, therapy and evidence-based care
Work confidently within a multidisciplinary team
Communicate clearly with clients, carers and colleagues
Manage priorities and maintain accurate, timely records
