We're looking for a compassionate and organised person to join our Housing Relocation Support team.
This Program Supports Residents Of Public Housing High-rises Being Relocated To New Residences. Working Alongside a Community Health Nurse And Occupational Therapist, You Will
- Engage directly with residents to understand their needs
- Coordinate referrals to health, wellbeing, and social services
- Provide practical non-clinical support to help residents settle into their new homes
- Build relationships with local services and promote the program
