About the Role
The Health Professional will lead the Family Health and Wellbeing Team, delivering the Children's Ground Family Health and Wellbeing Framework.
This framework improves health and wellbeing outcomes for individuals and families in local communities.
The team will work proactively with internal and external partners to develop and implement family health plans and responses to population health needs.
Key Responsibilities
* Health and Wellbeing Framework Delivery: Deliver the Children's Ground Family Health & Wellbeing Framework to improve health and wellbeing outcomes.
* Population Health Management: Develop, assess, and monitor population health/health promotion priorities and manage responses for improved health outcomes.
* Team Leadership: Manage the health team in a culturally appropriate manner.
* Integration and Planning: Support the integration of health and wellbeing into early childhood curriculum and develop family health and wellbeing plans.
* Social Support: Support local First Nations people to navigate public, not-for-profit, and private health systems.
* Infectious Disease Prevention: Oversee infectious disease prevention and response work.
* Outreach Programs: Design, support, and maintain outreach programs for the region.
* Clinical Partnerships: Establish relationships with clinical health providers and educators.
* Performance Monitoring: Monitor and report on data to meet evaluation needs.
* Staff Management: Recruit, induct, review performance, and mentor health staff.
