Job Title: Community Care Specialist

">
">
* The role involves carrying out initial, core and other specialized assessments as per national guidelines to formulate support plans for service users.
">
* Manage cases varying in complexity, organize packages of support to service users, assess levels of risk to their health and wellbeing and monitor and review their situations.
">
* Work directly with family members providing advice and guidance. Work to empower service users, promote their participation in planning and evaluation of service to increase their levels of self-sufficiency.
">
* Attend and participate in multi-disciplinary teams and meetings, planning reviews with colleagues and relevant professional staff to assist and analyze the needs of service users.
">
* Give written/verbal evidence in court/adoption/fostering panels when required to do so, carry out all necessary planning and assessments related to proceedings.
">
* Implement actions agreed by the courts to ensure the safety and wellbeing of service users within the specified timeframe.
">
* Demonstrate awareness/understanding of equal opportunities and other people's behavioural, physical, social and welfare needs.
">
* Ensure that reasonable care is taken at all times for the health, safe and welfare of yourself and other persons.
">

Key Responsibilities:

Requirements:

* Relevant qualifications and experience in social work or a related field.

* Excellent communication and interpersonal skills.

* Able to work effectively in a team environment.

* Ability to maintain confidentiality and handle sensitive information.

* Able to work under pressure and manage multiple priorities.

* Strong analytical and problem-solving skills.

* Knowledge of national guidelines and policies related to social work.

* Ability to work independently and as part of a team.

Working Hours: Full-time (Monday-Friday, 9am-5pm)
