POSITION DETAILS
The Recovery and Wellness Program (RWP) is a psychosocial outreach support program for people who have a severe and complex mental illness and reside in boarding houses, supported accommodations and crisis accommodation including (couch surfing). The Recovery facilitator provides one to one support to assist the program participant to design and work towards achieving their goals through engaging in a recovery-oriented practice framework. The RWP works in partnership with the Metro North and Metro South Hospital and Health Services. All program participants are referred by clinicians through Queensland Health's Mental Health Services.
KEY RESPONSIBILITIES
* To build rapport and relationships with individuals from a range of different backgrounds and support needs.
* To have knowledge and experience in Mental Health Recovery Principles, Strength-based and Person-centred practice, Trauma Informed Practice and Crisis Intervention model.
* Demonstrated experience and understanding of the provision of support services to people with severe mental illness and those at risk of homelessness.
* Ability to conduct ongoing comprehensive assessments to understand the program participants needs, barriers, risks, and vulnerabilities.
* Support the program participants to design their recovery goals and progress towards achieving these.
* Engage in psychosocial support and education to assist the program participants in making their own decisions and exercising self-determination.
* Support the program participant to access appropriate specialist services that meet their needs and longer‑term supports such as the National Disability Insurance Scheme and Aged Care services where appropriate.
* Engage in effective communications and collaboration with the program participants and the stakeholders who are involved in their recovery journey.
* Able to hold a caseload and manage appointments and administrative tasks in a busy working environment.
* Able to work effectively in a team environment and in lone working.
* Participate in case conferences and interagency meetings in a collaborative and consultative manner.
* Comply with reporting requirements by collecting, collating, and communicating all relevant data to the appropriate internal and external stakeholders as required.

* Participate in supervision and ongoing learning and development opportunities.
* Promote and maintain a safe and healthy workplace, ensuring adherence to WHS policies and procedures.

KEY SELECTION CRITERIA
* Minimum Diploma level qualification in a relevant discipline or significant progress towards such a qualification.
* Knowledge of Homelessness, AOD, Community Services and Health Sectors.
* Ability to show initiative, think critically and make judgments based on sound reason and the information/evidence presented.
* Good written and verbal communication skills, and computer literacy
* Ability to use a personal vehicle
* National NDIS Workers Screening check or current Yellow Card.
* Current First Aid and CPR Certificate
