This rewarding role involves shift work, and you will need to be available to work flexible shifts that may include a combination of mornings, evenings, and weekends.
Key Responsibilities
* Developing individualised support plans that incorporate customers' identified well-being needs and goals.
* Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication, and personal care.
* Providing psychosocial support to customers to explore their practical and complex needs.
* Maintaining accurate customer records using online systems.
* Developing and maintaining professional relationships and networks with customers, families, caregivers, and key stakeholders.
* Enhancing the customer journey through innovative approaches.
Required Skills and Qualifications
To excel in this role, you will require:
* Tech-savviness with proven ability to use Microsoft Office and reporting systems.
* A minimum Cert IV in Mental Health or Disability or equivalent industry experience.
* The flexibility to work across a rotating roster.
* A willingness to use personal mobile devices for work purposes.
* A valid NSW Drivers C class license, with the ability to drive vehicles in various classes.
* The willingness to obtain an Australian Federal Police Check, WWCC, and NDIS WC to meet employment criteria.
