Rare opportunity to work with one of our community clients and be part of a team that makes a difference. As a Community Registered Nurse, you will be working on a permanent full-time basis, enjoy consistent work in the community and visit clients in their own homes under aged care packages. Bring your passions for championing individualised care that keep consumers living independently in their own home. You will enjoy being part of an inclusive team working with the broader home and community team, business manager and scheduler and community support workers. They have an outstanding reputation, are highly valued in the community sector and are committed to delivering person-centred care and best practice to their 60 clients located Perth South. They provide in-home care and support, nursing support, post-hospital support, in-home dementia care, comfort calls and palliative care.
Day to day duties will include:
* Conducting client care assessments
* Supervision and management of support workers
* Coordinate the provision of quality person-centred care according to the client's needs, service/care plans and available funding in accordance with budget
* Clinical in-home services include care plan assessment, medication, incontinence, chronic disease, complex wound management, palliative care, and diabetes
* General administration with respect to tasks performed
* Developing, implementing and evaluating plans of care alongside the clients and their families
* Liaising with healthcare professionals to deliver quality care to clients
* Ensuring full compliance with organisation and care standards
To be successful:
* Current AHPRA registration
* Experience working in Home and Community Care or Aged Care
* Strong clinical assessment skills
* Excellent engagement and communication skills
* Amazing customer service, friendly approach, compassion and values focused
* Current WA driver's licence
* National Police Clearance within the last 6 months
* Current CPR and First Aid Certificate or willing to obtain
* Working rights in Australia
