Mūrrūmba Targan Djimbulung (MTD) provides wrap-around health and well-being support to First Nations adults who live in West Moreton and experience chronic health issues, such as diabetes and ongoing heart and lung issues.

About you
Our ideal candidate will be someone who can demonstrate the following:
* Demonstrated advanced clinical knowledge in the area of the chronic condition management delivering expert nursing care in complex situations while providing support and direction to other team members.
* Demonstrated ability to role model excellent standards of comprehensive, evidence-based nursing care and documentation to facilitate quality patient outcomes, in accordance with National Safety and Quality Health Service and professional standards.
* Demonstrated ability to communicate effectively and maintain a positive workplace culture through role-modelling well developed consultation, liaison, negotiation and conflict resolution skills, engaging professionally with members of the healthcare team, patients, relatives and visitors.
* Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use Virtual Care, digital health/information and reporting systems appropriate to the position.
* Demonstrated ability to communicate effectively with Aboriginal and Torres Strait Islander people and their families and communities to maintain a positive workplace culture through well developed consultation, liaison, negotiation and conflict resolution skills.
Mandatory Requirements:
* Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
