NDIS Support Coordinator

This role plays a vital part in assisting individuals with disabilities to navigate and access necessary services, supports, and resources within their community.


Key Responsibilities:

* Support Coordination and Case Management
* We work closely with NDIS participants to understand their needs, preferences, and goals, providing individualized support plans that align with these requirements.
* The role involves developing and implementing individualized support plans, collaborating with participants and their families or carers to ensure seamless service delivery.
* Ongoing coordination and monitoring of services and supports are essential to guaranteeing that participants' objectives are being met and they are satisfied with the services provided.
* A comprehensive understanding of the NDIS funding process is crucial, ensuring that participants' budgets are managed effectively.
* Timely resolution of any issues or barriers to service delivery is also a key responsibility.
* Liaison and Stakeholder Management
* We foster positive working relationships with NDIS participants, service providers, and other key stakeholders, advocating on behalf of participants to secure the best possible services.
* Collaboration with healthcare professionals, therapists, and other service agencies ensures the holistic support of participants.
* All services provided must be aligned with NDIS plan goals, standards, and guidelines.
* Monitoring and Reporting
* We monitor and track progress of NDIS plans, ensuring timely reviews and amendments as required.
* Accurate and up-to-date case notes, reports, and documentation are maintained in line with organizational policies and NDIS requirements.
* Education and Empowerment
* We educate participants and their families on how to navigate the NDIS system and make informed choices about their supports and services.
* Empowering participants to become confident and independent in managing their NDIS plans and connecting with the services they need is a key objective.
* Compliance and Quality Assurance
* We ensure all work complies with the NDIS Code of Conduct, relevant legislation, and organizational policies.
* Contributing to continuous improvement efforts by providing feedback and participating in training and development opportunities is essential.
