About us:
The Geriatric Emergency Department Intervention (GEDI) focusses on the frail older person presenting to an Emergency Department (ED) with an acute illness or complex healthcare requirements. Usually this incorporates people of 75 years of age and over. However, frail older persons who are under this age and Indigenous Australians 55 years of age or older, who may have similar levels of frailty, are also screened by the team and may be included in service delivery. The GEDI model is aimed at improving the quality of care for this cohort, reducing unnecessary hospital admissions and facilitating early safe discharge.
About the role:
The GEDI Clinical Nurse (CN) provides an advanced practice role within a multidisciplinary approach to the screening, assessment and care coordination of the aging population presenting to the ED to ensure appropriate and coordinated care of identified vulnerable patients. The CN will have demonstrated enhanced inter-professional and client/carer communication, as well as sound care planning of the elderly.
About you:
Current registration through the Australian Health Practitioner's Regulation Agency (AHPRA).
While not mandatory, a relevant postgraduate certificate or post graduate diploma is an expectation of this position.
* Where the successful applicant does not currently hold this qualification, commitment to completion within an agreed timeframe is expected and will form part of the performance and development plan.
What we offer:
Total remuneration value of up to $131,900 p.a. is comprised of:
* An hourly rate between $ $58.30 p.h.
