Based in Toowoomba, you will support patients in their own homes and work closely with a multidisciplinary team to deliver person centred, evidence based services. What Your Day Will Look Like….
In this role you will deliver advanced clinical services to older inpatients within their home environment, using strong professional judgment and a commitment to evidence‑based practice. With Toowoomba Hospital as your base, you'll work across the community as part of a multidisciplinary HITH‑CAC team, shaping service delivery, guiding best practice, and contributing to high‑quality outcomes that reflect Darling Downs Health values. The position also offers scope for leadership, mentoring and service development, making it well suited to someone who is motivated by autonomy, continuous learning and meaningful connection with patients and colleagues.
