Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services. 
NHW have an exciting chance for a passionate and experienced staff member to join our team as a Sub-acute Ambulatory Care (SACS) Co-ordinator. 
In this vital role, you will provide day-to-day operational leadership and coordination across the SACS and Transition Care Program (TCP) services. The role is key to delivering high-quality, goal-directed care that supports clients\' independence, promotes hospital admission avoidance, and ensures smooth patient flow throughout Northeast Health Wangaratta (NHW). 
As SACS Co-ordinator, you will: 
- Support and guide staff in delivering holistic, person-centred care. 
- Work collaboratively across NHW departments and with external services. 
- Promote NHW as a leading regional health care provider. 
- Lead with compassion, initiative, and a strong team-based approach.

Our Region 
- Wangaratta has a population of approximately 30,000 and our catchment serves a community of around 100 000 throughout the northeast. 
- An easy 2 ½ hours drive on the Hume freeway from Melbourne and less than an hour to Albury airport with direct flights to most of the large Australian cities.
