Case manager supported tenancy for families program

Overview
This is an exciting new position at TAIHS.
The TAIHS Supported Tenancies service is a new service (established January ****) that works closely with the Department of Housing and Public Works to provide mobile support to young parents and pregnant women (young families) who are living in social housing and at risk of homelessness.
Case managers will use a home visiting model of support to develop and maintain strong relationships with the clients to support their housing arrangements.
Duties & Responsibilities
Assist young families to build parenting skills and confidence in caring for their babies and children, including helping to establish and maintain an emotional bond and to develop and maintain living skills and effective routines.
Connect and work with other services to meet the young families' social and cultural needs and help them move towards social and economic independence, including supporting them to engage in education, training and employment.
Assist young parents to engage with different support services and connect with relevant community agencies to build a robust safety and support network for young parents and their children.
Build and support young parents' capabilities and connections using a culturally holistic case management approach.
Provide information support and advocacy to young parents.
Monitor client needs, identify risks, develop case plans and review.
Develop and maintain effective and respectful relationships with clients and provide nurture and support for young parents while assisting in the development of their skills and build capacity to maintain tenancy and meet the developmental needs of their child.
Complete referrals and send them to internal and external services with consent provided and based on the identified needs of the family.
Participate in all aspects of support and case management practice as per the Human Services Standards.
Liaise with the Line Supervisor in relation to all client-related matters, including client needs, engagement, issues, observations or concerns.
Submit reports to the Manager / Practice Lead in accordance with reporting schedules and directions.
Ensure secure management of all program files and compliance with applicable privacy legislation and policies.
Develop and document good working relationships and/or collaborative partnerships with relevant agencies.
Actively participate in scheduled community-based activities to provide brief interventions for information, advice and referral and promote and support families' participation in evidence-based community programs.
Required Qualifications & Experience
Minimum Cert IV Child Youth and Family Intervention or equivalent.
Hope and Healing qualification is desirable.
Demonstrated experience working with and engaging Aboriginal and Torres Strait backgrounds and an understanding of cultural competence.
Developed knowledge of child development and parenting skills models and theory, attachment theory and strengths-based service delivery.
Ability to operate within a case management model with strong capabilities around building relationships with and motivating vulnerable people, assessing needs and advocating for the client.
Proven understanding of the historical, social, cultural and economic issues affecting Aboriginal and Torres Strait Islander communities.
Competence in the use of business technology and desktop applications: Internet, Word, spreadsheet and database packages.
High level communication skills (written, verbal and interpersonal) when liaising with clients and Departmental representatives.
Required Licences and Checks
Ability to pass a National Police Check (no serious/criminal/court records).
Current Blue Card (Queensland Working with Children Check).
Current Class C Driver's Licence (Queensland).
Other Requirements
Proof of qualifications and current registration (as appropriate) are to be provided prior to commencement of employment.
Ability to perform the physical requirements of the role in a safe manner.
Offer of the position may be subject to a pre-employment medical assessment.
