Job Description
About the Role
The NDIS Support Coordinator supports participants of the National Disability Insurance Scheme (NDIS) to implement the supports outlined in their NDIS Plan and achieve their NDIS Plan goals by coordinating:
* Informal supports e.g. connecting with family, friends and other community groups or networks
* Funded Supports e.g. liaising with providers to deliver supports outlined in the participants NDIS Plan
Additionally, the NDIS Support Coordinator will assist participants to:
* Understand their plan
* Identify and connect with services and providers to help them achieve their goals
* Arrange assessments that may be required
* Develop skills and build capacity to self-coordinate their supports
Note: This role is responsible only for Coordination of Supports and does not include Support Connection or Specialist Support Coordination (see NDIS Pricing Arrangements and Price Limits for further information).
Key Responsibilities
* Provide coordination support to NDIS participants and their families / carers where relevant. This includes informal, mainstream, and funded supports to assist them in implementing the supports outlined in their plan
* Ensure that all service provision options are presented to participants in a clear, accurate and un-biased manner in order to support them to exercise choice and minimise any potential for conflicts of interest
* Support participants to develop skills and capacity in understanding their NDIS plan and supports, using the participant portal, developing service agreements, monitoring service provision and liaising with providers as needed
* Meet targets for billable support coordination hours as set out by the program
* Prepare and submit reports to the National Disability Agency on progress and outcomes within the agreed reporting timeframes
* Liaise and work with NDIA planners and Local Area Coordinators, health and other support providers as relevant to facilitate plan implementation

Role Requirements
Essential
* Tertiary qualifications in Disability, Social Work, Health, Welfare or equivalent discipline, OR
* Certificate IV in Disability Services or similar and knowledge and experience within the sector
Critical to your success in this role will also be:
* Demonstrated experience of working with people with a disability and / or in coordinating services for people with disabilities
* High-level engagement skills and ability to establish empowering and positive partnerships with participants and their families
* Understanding of relevant legislation including the National Disability Insurance Scheme Act 2013, Disability Act 2006 and the NDIS operational guidelines
* Willingness to obtain a valid Working with Children Check, NDIS Worker Screening Check and satisfactory National Police Check prior to commencing employment with us. If you have spent over 12 months overseas in the last 10 years, you will also be required to undergo an International Police Check or present evidence of a satisfactory check
* Valid VictorianDriver Licence
