Role Overview:
The Clinical Care Coordinator will contribute high-quality evidence-based clinical care as a member of the multidisciplinary consortia team delivering care coordination services for children and youth, focusing on early intervention and support of young people up to the age of 25.
This role is part of the initiative aimed at delivering comprehensive, integrated, and person-centred care across the region. The Clinical Care Coordinator will address service gaps for the 'missing middle' by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.
Key Responsibilities:
* Contribute high-quality evidence-based clinical care as a member of the multidisciplinary consortia team.

* Promote holistic wellbeing by considering broader determinants of health, fostering collaboration among providers, and ensuring a sustainable, adaptable service responsive to community needs.
Required Skills and Qualifications:
* Must have one of the following: Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, OR, AASW registration/eligibility as a Social Worker, OR, Level 2 ACA registration/eligibility.
* Knowledge and understanding of the theory underpinning evidence-based interventions and clinical practice as well as demonstrated excellence in the practical application of such, in the areas of mental health and alcohol and drug use.
* Highly developed communication (including written) and interpersonal skills.
* The ability to engage with people, advocate on their behalf and establish and maintain effective relationships with staff, clients, families & friends and stakeholders.
* Experience in the use of electronic client/customer management systems, ability to use a desktop computer, and experience with Microsoft Office applications.
* Must be eligible to apply/currently hold a Blue Card for paid employment.
* Must hold/be willing to apply for NDIS worker screening clearance for paid employment.
