The position will work collaboratively with Aboriginal Health Workers as well as social work to provide assessment, care coordination and education to families and take into account clinical, psychosocial and cultural needs.The program works with women with an Aboriginal baby and their families, from the antenatal period until the child is 5 years of age. This will include home visiting and practical support based on primary health care principles to promote a safe and secure environment for children.
