In this role, you will lead the delivery of community-based care, working collaboratively with multidisciplinary teams to provide high-quality support to clients. A typical day may involve:
1. Supporting Clients: Provide meaningful support to clients in their homes or clinic spaces, tailoring your approach to meet their unique needs.
2. Preparing for Appointments: Ensure seamless communication with clients and healthcare professionals through effective documentation and reporting.
3. Collaborating with Teams: Work closely with Clinical Supervisors, Team Leaders, and other healthcare professionals to deliver comprehensive care.
