The responsibilities of the position include:Health care planning: Develop and monitor personalised care plans, including complex needs. Ensure external HI plans comply with NDIS standards.Training and support: Develop and conduct education with staff in NDIS-aligned care. Assess supervisors' competency in medication management.Care coordination and monitoring: Conduct assessments and monitor care plans. Track medical incidents and medication errors.Clinical practice leadership: Lead clinical practices and guide support workers and service managers. Promote health and support preventive care initiatives.Collaboration with teams: Work closely with teams to ensure high-quality care and compliance with NDIS standards.
