**About the Role**

Join our team as a Grade 2 Social Worker on a part time (0.6EFT) ongoing basis. This position will work across all of Monash Health sites.

Your role as the Grade 2 Social Worker will be providing psychosocial assessment and intervention, coordination of care across the continuum and intervention for clients who have complex health issues/ needs. You will work in collaboration with the multidisciplinary team and be responsible for holding a clinical caseload which will involve assessment, care coordination, systems navigation, secondary consultation, service development and innovation.

Your aim is to ensure that the clients and families have information and support, as required, to access a range of formal and informal supports. The program utilises an assertive outreach model in line the programs guidelines providing home visits as appropriate to clients residing in the Monash Health catchment.

**About Us**

The Primary Care Support Team, as part of the Community Assessment and Response (CART) directorate offers a variety of programs that assist individuals to manage chronic health conditions, focusing on the assessment and delivery of client focused care. The team consists of Allied Health Assistants, Care Coordinators, Community Development Workers, Counsellors, Diabetes Nurse Educators, Dietitians, Exercise Physiologists, Leisure & Lifestyle Assistants, Occupational Therapists, Physiotherapists, Recreational Therapists, Social Workers, and Podiatrists. The team is committed to working with communities, families and individuals who have poor health outcomes and to providing high quality, multidisciplinary programs which embrace client centred care, evidence based practice and the Social Model of Health.

**About You**

With extensive experience supporting individuals and families through psychosocial assessment, crisis intervention, and complex discharge planning, you thrive in multidisciplinary teams and understand the critical role Social Work plays in navigating social complexity and vulnerability. You are confident working with clients across the lifespan and demonstrate cultural sensitivity and ethical integrity in all interactions.

You bring a depth of knowledge in key areas including family violence, elder abuse, child protection, NDIS, and liaising with statutory and community agencies. You are skilled in managing fluctuating workloads, prioritising urgent needs, and maintaining accurate clinical documentation in accordance with health service procedures.

A natural leader and mentor, you enjoy guiding students and junior staff, supporting professional development, and contributing positively to team culture. You are proactive in managing change and fostering innovation to improve service delivery. As a key contributor to Monash Health's Social Work Service, you are ready to make a meaningful difference to the lives of vulnerable people in your community.
